Any idea, looks like a manhole cover but it next to a large concrete slab.

Some sort of sewage perhaps?

Is it old? Could the "MWB" refer to an old water board, perhaps?

The large concrete cover may hide a nest of water valves.

BTW to OP, MBW would be Metropolitan Board of Works if you find an anagrammatical manhole cover.

Clearly they hadn't yet learnt that the problem with large rectangular manhole covers is that oiks can drop them down the hole. Not good for septic tanks.

Not just a british hobby either - There is a minor Japanese obsession with "manhoru". I bought SWMBO this book for Christmas a few years ago (who says romance is dead!)

At the risk of a serious answer...

If it were around here it might well be an old fashioned "concrete box" septic tank (or cess pit).
